Complete list of all medications and dosages you are currently taking.
Results of most recent HA1C if you are diabetic.

If your insurance company requires prior authorization,
please bring your authorization form.

Please arrive approximately 10 minutes before your appointment and if applicable,
bring any current glasses or contact lenses.
